Worms routinely bypass perimeter firewalls and gateway AV scanners, sneaking into corporate networks on trusted, but compromised mobile clients. Users infect their LANs by jacking infected laptops into the network or connecting via a VPN through the firewall. What enterprises need is end-to-end security that checks and enforces a policy of trusted or untrusted status before granting access.
